At the same time, for many people, it remains unclear what berberine actually does, and why one capsule isn't like another.

What is berberine and how does it work in your body?
Berberine is a bright yellow alkaloid naturally found in the roots and bark of plants such as barberry (Berberis vulgaris) and Indian barberry. It has been traditionally used for centuries and has recently been extensively researched, particularly in relation to blood sugar levels and metabolism.
The AMPK pathway in plain language
Berberine's most discussed mechanism of action is via an enzyme called AMPK. You can think of AMPK as a kind of energy sensor in your cells: it becomes active when the cell signals that attention is needed for energy management. Berberine gives that sensor a nudge. As a result, it plays a role in processes related to glucose metabolism, fat metabolism, and insulin sensitivity.
Interesting detail: this is precisely why berberine is so often mentioned alongside longevity ingredients. AMPK is the same pathway that is also activated by physical activity and by metformin, albeit through different routes. Research suggests that berberine activates AMPK slightly differently and more gently than metformin, but the underlying idea is similar.
Why berberine has a bioavailability problem
This is where the biggest practical challenge lies. Standard berberine has remarkably low bioavailability: in its raw form, it's estimated that less than 1 percent is actually absorbed by the body. The rest largely passes through unused.
This doesn't mean that standard berberine doesn't work, as it is used in countless studies in that very form. However, it does mean that bioavailability is a central factor in choosing a supplement, and this is precisely where the different forms vary. We'll come back to this later.
Berberine and metformin: a fair, neutral comparison
No discussion about berberine can escape the comparison with metformin, the widely used drug for type 2 diabetes. This comparison is understandable, as both impact glucose metabolism through partly overlapping mechanisms.
What does the research say? A meta-analysis of 46 studies with over 4,000 participants found that berberine and metformin showed comparable effects on blood sugar levels. On paper, this is a striking result. At the same time, nuance is needed here. Berberine research often consists of smaller, shorter studies, while metformin has been investigated for decades in large-scale, long-term trials. Furthermore, metformin is a registered medication requiring medical supervision, and berberine is a dietary supplement. They are therefore not interchangeable entities.
It's important to be clear: berberine is not a substitute for prescribed medication. If you are using metformin or other medications, always consult your doctor before adding anything to your routine.
David Sinclair's point: timing and cyclical use
Longevity researcher David Sinclair has popularized the idea that berberine can be seen as the natural counterpart to metformin because it acts on a similar pathway. There's an interesting practical point attached to this that's worth understanding.
Both berberine and metformin activate AMPK. But exercise does too, via its own route. There are indications that AMPK activators around training can blunt training adaptation, in other words: precisely the signal you want to strengthen during exercise can be partially weakened if you layer an AMPK activator over it. From that reasoning, some choose not to take berberine on a training day, and for example, use it every other day. A similar cyclical approach is seen with ingredients like resveratrol.
This is not a prescription and not literal advice from Sinclair, but a framework that shows that timing can be relevant with these types of ingredients. What suits you depends on your goals, your lifestyle, and ideally your own blood values.
"Nature's Ozempic"? Why that comparison falls short
The nickname "Nature's Ozempic" has brought berberine a lot of attention, but the comparison is more marketing than mechanism. It's a good example of a claim that deserves to be nuanced.
Ozempic and similar drugs act as so-called GLP-1 agonists: they mimic a gut hormone that, among other things, affects feelings of satiety. Berberine works via a completely different mechanism, namely primarily through the AMPK pathway we discussed earlier. Different points of action, different effect sizes, and different evidence. Berberine does not approach the effects of this class of drugs in research.

Bioavailability: why the form matters
Here we return to the bioavailability problem, as this is often the practical difference between the forms. If less than one percent of standard berberine is absorbed, the logical question is: can it be improved?
It can. A phospholipid or phytosome form encapsulates the berberine in a way that makes it easier for your body to absorb, in pharmacokinetic research up to about ten times more than unprocessed berberine at the same dose. At the same time, there are clear situations where berberine is not recommended. It is not recommended during pregnancy or breastfeeding, and not for young children. If you are taking medication, especially drugs that affect blood sugar, always consult your doctor first, as berberine can affect their action. Caution is also advised if you have low blood pressure.
Good to know: berberine is currently being evaluated by the European Food Safety Authority (EFSA) in an ongoing safety assessment of botanical ingredients. A definitive judgment on its safety is therefore not yet available. This is an additional reason to use it consciously: if you have doubts, are pregnant or breastfeeding, or are taking medication, consult your doctor first.
Regarding dosage, research typically uses around 500 mg per day, often divided throughout the day and preferably with meals; otherwise, adhere to the instructions on the product. In terms of side effects, berberine is generally well-tolerated: at higher doses, mild gastrointestinal complaints may occur, such as bloating or altered bowel movements, which often decrease by lowering the dose or taking berberine with meals.
